Anthem Indiana Health Insurance Plans

Anthem Premier Plus

The Premier Plus plan is Anthem’s most comprehensive policy. Doctor’s office visit coverage (number of visits) is unlimited with a $30 copay for primary care physician visits and a $40 copay for specialist visits. This coverage also can be removed,if not needed.

Two prescription benefits are available (Standard and Upgrade). The standard benefit includes a $250 deductible (per person) for brand-name drugs for Tiers 2, 3 & 4. The upgrade benefit provides a broader coverage and potentially lower out-of-pocket costs on expensive drugs.

Major medical deductible options for this Anthem Indiana plan range from $500 to $10,000 with a 20% coinsurance (capped). A 0% coinsurance option is also available with the higher deductibles. Naturally, higher deductibles will substantially reduce the rate. We feel that the $5,000or $2,500 deductible with 0% coinsurance is the most cost-effective.

Nationally recommended preventive services are covered at 100% with no waiting period, deductible or coinsurance. Some of these services include Pap tests, mammograms, annual routine physicals, immunizations, well-child care and PSA screenings.

Anthem SmartSense

The SmartSense plan provides coverage for three office visits per year to a primary care physician or specialist. A $35 copay applies and any additional visits are subject to the plan deductible. However, those visits will receive favorable Anthem Network discounts.

A “Standard” and “Upgrade” prescription benefit is available. If the “Standard” option is selected, Non-Formulary drugs will only receive the Anthem negotiated discount. The “Upgrade” benefit includes a combination of copays and coinsurance, depending on the type of drug.

Preventive benefits are also included at 100% coverage with no waiting periods, copays or coinsurance.  Unlike the Premier Plus plan, maternity benefits are not available as a rider. However, dental and vision riders are available on both plans.

The Anthem Indiana SmartSense plan costs substantially less than the Premier Plus plan and is recommended for persons that want affordable coverage without some extra benefits that may not be used. Depending on the size of your family, the savings could be thousands of dollars each year.

Anthem Lumenos HSA

One of the most popular Indiana health insurance policies is the Lumenos Health Savings Account (Indiana HSA) plan. Like all HSAs, this type of coverage is designed to provide much more control over your health care costs and also pay for qualified medical expenses with tax-favored dollars. The HSA portion of the account may be set up at the time of application, or at a later datewith your choice of banks. The policy also functions without the HSA side account. 

Once again, qualified preventive benefits are covered at 100% and not subject to a deductible. Once any combination of listed policyholders meets the deductible, the plan will pay all of the covered expenses. Individual deductible options range from $1,500 to $5,000. Family deductible options are $3,000 to $11,000.

Anthem CoreShare

The Coreshare plan is an inexpensive plan that also has higher out-of-pocket costs for many covered benefits. Coinsurance for many services is 50%. Also, a facility copayment applies to outpatient surgeries and inpatient hospital visits. Higher deductibles will not include this extra charge.

Although qualified preventive benefits are covered at 100%, non-preventive office visits are subject to a deductible. A $25,000 deductible is available with a 0% coinsurance option. In early 2012, a "Deductible Credit" program was introduced. The program creates a special account that that rewards you when you don't meed your deductible. Plans that have a $2,500 or higher deductible ($3,000 for a Lumenos HSA) will qualify. You must have been insured under the plan for more than three months and there are a few other requirements.
